RIDES Assistance


Copies of the form you submit will be sent to ProMed Delivery Customer Service and to MCLS.  Please include as much information as possible.  For complete instructions, see RIDES Procedures.

You will be asked to choose ONE of the following options available on the form:

Damaged Material -- If your items or packages arrive damaged. Reimbursement requests are to be submitted by the Borrowing Library. Please submit one form per item, and click to open the details area.

Lost/Missing Material -- If you believe items or packages are missing from a shipment.  This request is to be submitted by the Shipping Library. Please submit one form per item, and click to open the details area.

Non-Delivery -- If your normal delivery service has not occurred. Enter information in the Additional Information box.

Closing Information -- If your library will be closing hours other than your normal schedule. Enter information in the Additional Information box.

Miscellaneous -- Label maker issues, or any other issues that do not fit the above criteria. Enter information in the Additional Information box.

When you are submitting an email to RIDES Assistance, be sure to click only ONE of the service issues.  For example, if you complete the 'Lost/Missing' information, but then you click on 'Miscellaneous', your text will not appear in the email.  If you have more information to add to a 'Lost/Missing' report, please enter it in the 'Additional Information' free text field.
